MEAL PROGRAMS COORDINATOR
(Starting: $40,950 - $44,460)
Southern Frontenac Community Services is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.
The Meal Programs Coordinator is responsible for the hot and frozen Meals on Wheels Program to provide healthy meals, wellness checks and social engagement for rural seniors. The Meal Programs Coordinator manages volunteers to deliver the programs and works closely with the Cooks to contribute to meal planning.
This is a full time (35 hours weekly) position, based in Sydenham, ON.
Primary Duties:

Plan, schedule, promote and carry out Meals on Wheels program activities.

Provide training direction to a team of volunteers. Assist with recruitment.

Communicate regularly with clients.

Collaborate with staff and network partners.

Maintain and report statistical data.

Enter all client and order information, maintain accurate data for billing purposes.

Maintain and nurture community partnerships.

Requirements:
-
Strong awareness and/or lived experience that demonstrates knowledge and compassion of the challenges of seniors and vulnerable people in a rural environment.
-
Post-Secondary education relevant to social and community services and/or healthy aging with strong administrative component.

Experience leading and mentoring.
-
Be an organized, self-starter with demonstrated ability to meet deadlines.

Strong computer skills and proficient with MS Word, Excel, Outlook. (Will be tested) Ability to learn new applications.

Ability to perform the physical aspects of the role such as lifting supplies/food products, set up/take down of tables and chairs.

Provide own transportation to travel to off-site program locations. Mileage reimbursed.

CLIENT ENGAGEMENT SUPERVISOR
(Starting: $30.00 - $32.65/hour)
Southern Frontenac Community Services (SFCSC) is a rural based organization focused on working with others to help seniors age well in their own community and to facilitate connections and provide families, including seniors, with services to meet their basic needs.
The Client Engagement Supervisor facilitates the onboarding of new clients referred into SFCSC seniors’ community care programs in a way that ensures the best possible outcomes. The Supervisor regularly communicates with internal and external partners, provides oversight to client data management systems and ensures coordination of care services to enhance the overall client experience and wellbeing.
This is a full-time (35 hrs/week), indefinite position. One vacancy is available.
Primary Duties:

Facilitate new client introductions to programs in a warm manner.

Provide direct supervision and coordination of the In-Home Help Programs, including staff supervision, training and scheduling.

Be a liaison with external community service partners for new client referrals.

Communicate regularly with any referral or partner agency including (but not limited to): physician offices, discharge planners, SMILE, Regional Care Coordinators and self-referrals.

Provide oversight to client data management systems including NesdaTrak, Caredove and SHIIP in accordance with personal and health data privacy regulations.

Collaborate with staff to facilitate services.
-
This role has a heavy administration component and requires strong organization and computer skills with a workflow improvement mindset.
Qualifications & Requirements:

Post-Secondary education relevant to seniors care and/or social services e.g. Gerontology, Social Service Worker, RPN etc.

Must have strong computer and organizational skills and demonstrated experience in MS Office suite including Word, Excel, Outlook (will be tested). Experience in NesdaTrak, Caredove and SHIIP will be deemed an asset but not required.

Proven leadership and supervisor skills with a passion for seniors care and community.

Demonstrated ability to collaborate well with others and quickly develop positive rapport.

Ability to empower individuals, in particular seniors, to live independently and confidently navigate life’s challenges by providing compassionate, knowledgeable and non-judgmental support.

Must have own transportation and valid license and insurance.
