Meal Programs Coordinator

(Starting $40,950 - $44,460)

Southern Frontenac Community Services is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.

​

The Meal Programs Coordinator is responsible for the Meals on Wheels and Diner’s Program to provide healthy meals, wellness checks and social engagement for rural seniors.  The Meal Programs Coordinator manages volunteers to deliver the programs and works closely with the Cooks to contribute to meal planning.

​

This is a full time (35 hours weekly) position, based in Sydenham, ON. 

​

Primary Duties:

  • Plan, schedule, promote and carry out Meals on Wheels program and diner’s club events

  • Provide recruitment, training and direction to a team of volunteers

  • Communicate regularly with clients

  • Maintain and report program budget and statistical data

  • Maintain and nurture community partnerships

Requirements:

  • Strong awareness of the challenges of seniors and vulnerable people in a rural environment.

  • Experience leading and mentoring.

  • Be an organized, self-starter with demonstrated ability to meet deadlines.

  • Strong computer skills and proficient with MS Word, Excel, Outlook.  (Will be tested) Ability to learn new applications.

  • Ability to perform the physical aspects of the role such as lifting supplies/food products, set up/take down of tables and chairs

  • Own transportation to travel to off-site program locations. Mileage reimbursed.

  • Post-Secondary education relevant to the role.

​​Home Help Provider – Casual

($20.60 - $21.50 an hour)

Southern Frontenac Community Services (SFCSC) is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.  This is a casual position, Monday through Friday, 8:30 a.m. – 4:30 p.m., located in Sydenham, ON

Duties:

  • Work closely with the Client Care Coordinator, to perform household management tasks for seniors and adults living with disabilities enabling them to remain in their homes longer.

  • Work with client or their representative to ensure understanding of the tasks they are looking to you to provide or support.  These may include (but not limited to): cleaning any room of a home, washing floors, windows, dishes, bathrooms, laundry, meal preparation, or errands (e.g. retrieving mail or grocery items). 

  • Complete requested tasks in a thorough and efficient manner, being respectful of the client and the client’s property and sensitive to their level of comfort with you in the home.

 

Qualifications & Requirements:

  • Experience providing house cleaning or household management services to others.

  • Experience interacting with seniors.

  • Two positive references.

  • Must have own transportation.

  • Commitment to comply with SFCSC confidentiality agreement and values.

  • Willingness to attend training or meetings as requested.

  • Work with client or their representative to ensure an understanding of the responsibilities/tasks they are looking to you to provide or support while in the home.

  • Complete requested tasks in a thorough and efficient manner, being respectful of the client and the client’s property and sensitive to their level of comfort with you in the home.

 

Qualifications & Requirements:

  • Personal Support Worker Diploma or Recreation and Leisure diploma or equivalent from a recognized institution.

  • Experience providing personal care and household management services to others.

  • Two positive references.

  • Must have own transportation.

  • Commitment to comply with SFCSC confidentiality agreement.

  • Willingness to attend training or meetings as requested.

Foot Care Service Provider

(Contract - $25/hour)

​

Southern Frontenac Community Services (“SFCSC”) is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.

​

The Foot Care Provider is a contracted service worker engaged by SFCSC to provide basic foot care services to seniors living in South Frontenac at clinics held by SFCSC on a monthly basis.  The Foot Care Provider will provide a basic assessment, treatment and documentation with each appointment. 

This is a contract position. Work location is Sydenham, ON.

​

Primary Duties:

  • Attend clinics at SFCSC location(s) on the second and fourth Tuesday each month from 12:00 pm to 4:00 pm, dressed appropriately

  • Provide foot care services to clients following KFL&A Public Health and IPAC Practice Recommendations

  • Adhere and support sanitization practices between clients

  • Perform assessment and determine treatment plan for clients

  • Provide basic foot care including toenail cutting, callous reduction and application of lotion to each client

  • Document details of treatment provided, concerns and other relevant information in client file

Requirements:

  • Successful completion of a recognized Foot Care Certificate program

  • Must have own transportation

  • Successful completion of IPAC Core Competencies

  • Basic First Aid and CPR is and asset

  • Willingness to cooperate with any inspections of service

  • Willingness to attend training or meetings as requested

  • Proof of N-95 Mask Fitting

  • Proof of Full COVID-19 Vaccination
