Client Care Supervisor - Home Services
​​​(Starting $31.00 - $35.00/hr)

Southern Frontenac Community Services (SFCSC) is a rural based organization focused on working with others to help seniors age well in their own community and to facilitate connections and provide families, including seniors, with services to meet their basic needs.
The Client Care Supervisor facilitates the onboarding of new clients referred into SFCSC seniors’ home service programs in a way that ensures the best possible outcomes. The Client Care Supervisor regularly communicates with internal and external partners, provides oversight to client data management systems and ensures coordination of care services to enhance the overall client experience and wellbeing.
This is a full-time (35 hrs/week), indefinite position.
Primary Duties:

Client intake, assessment and care coordination for visiting volunteer hospice and palliative care, and/or staff provided home help services.

Oversight of service delivery, including supervising and supporting both staff and volunteer resources.

Timely completion of administrative tasks including documentation, case noting and statistical data collection, reporting and client file management using computer systems.

Collaborative work with internal staff and external community partners and contributing to organization’s leadership team.
Qualifications & Requirements:

Post-secondary education relevant to caring for aging adults and/or palliative populations (e.g. Gerontology, Nursing, Social Service Worker).

Supervisory experience, including coaching and providing performance feedback.

Strong computer skills and demonstrated experience in MS Office (may be tested). Experience with NesdaTrak, Caredove and SHIIP is considered an asset, but not required.

Proven leadership skills and a passion for seniors’ care and community.

Demonstrated ability to collaborate effectively with others and quickly develop positive rapport.

Ability to empower individuals, particularly seniors, to live independently and confidently navigate life’s challenges by providing compassionate, knowledgeable and non-judgmental support.

Must have own transportation and valid license and insurance.

Program Documentation Assistant
Part-time, Term Position (1 vacancy)
(Starting wage $22.00 - $24.95/hr)

Position Overview:
The Program Documentation Assistant (Program Assistant) supports the effective, safe and consistent delivery of food bank services by developing clear standard operating procedures (SOPs) for daily operations. Working closely with program staff and volunteers, the Program Assistant gathers information through interviews and direct observation and translates it into practical, user-friendly documentation aligned with organization values, safety standards and privacy expectations.
The Program Assistant also provides administrative and program support and offers front-line coverage for food bank inquiries when the program coordinator is off-site. The role may expand to include SOP updates in other program areas and additional administrative projects.
Hours: This role is a new part-time (25 hrs/week), 1-year term role with possibility of extension. Hours will be approx. 9:30am – 3:00pm with a 30-minute unpaid lunch break Monday – Friday.
Location: 4295 Stagecoach Road, Sydenham Ontario. This is not a hybrid or remote position.
Primary Duties:

Develop clear, step-by-step procedures, for food bank operations by interviewing staff and volunteers and observing core activities (intake, sorting storage, inventory, distribution).

Coordinate review and usability testing of SOPs as requested. Provide general administrative support, including scheduling, data entry, photocopying, correspondence and preparation of materials.

Provide coverage for food bank inquiries when the Food Bank Coordinator is off-site. Address food bank inquiries from volunteers and clients providing accurate information and prioritizing responses. Accept food donations according to established procedures.

Coordinate and/or assist with other administrative projects such as management of historical records, converting physical records to electronic format and general records management.
Qualifications & Requirements:
-
1+ years experience in an administrative role with program support and/or customer service (non-profit/community services considered an asset)
-
Strong writing skills; able to turn complex tasks into clear step-by-step instructions (with or without generative AI tools)
-
Strong listening/interviewing skills; comfortable gathering details from staff and volunteers
-
Highly organized with strong attention to detail and follow-through
-
Proficient in Microsoft 365 programs (may be tested)
-
Professional, respectful communicator with strong customer service skills
-
Commitment to dignity, inclusion, and confidentiality
-
Self-directed, collaborative and able to build rapport quickly
-
Valid driver’s license, insurance and reliable transportation

Additional Assets:
-
Comfortable in a food bank or similar setting (standing, light lifting, moving around the space)

Experience creating SOPs, process maps, checklists, or training materials

Familiarity with volunteer coordination and related practices

Food safety awareness and/or warehouse/stockroom experience

(Starting $20.60 - $22.35 an hour)
Southern Frontenac Community Services (SFCSC) is a rural based organization focused on working with others to help seniors age well in their own community and to facilitate connections and provide families, including seniors, with services to meet their basic needs.
This is a casual position requiring a minimum availability of approximately 20-25 hours per week, Monday to Friday 8:30 a.m. to 4:30 p.m.
Primary Duties:

Provide household management tasks for seniors and adults living with disabilities, including (but not limited to): cleaning any area of the home, washing floors, windows, dishes, bathrooms, laundry, meal preparation, or errands (e.g. retrieving mail or grocery items).

Complete requested tasks in a thorough and efficient manner, maintain a friendly presence while being respectful of the client, their needs and their property.
-
Observe and notify Supervisor and/or Coordinator of any changes in clients’ needs.

Maintain professional boundaries at all times and protect client confidentiality.
-
Complete relevant documentation and submit in a timely manner.
-
Attend office regularly for meetings and documentation submission.
Qualifications & Requirements:
-
Experience providing house cleaning or household management services in an efficient and safe manner.

Compassionate, dependable and client-focused.
-
Knowledge of needs of seniors and rural environment living.
-
Must have reliable transportation (mileage compensated) and have valid driver’s license and proof of vehicle insurance.

Must have ability to work independently and maintain professional boundaries.

Commitment to comply with SFCSC policies and procedures, including privacy requirements and to participate in training and staff meetings.

Flexible availability to provide services as scheduled, and attend training or meetings as requested.

Personal Support Worker certification considered an asset and will increase client assignment opportunities.
