Hospice Palliative Care Coordinator

(Rate of Pay: $24/hour - 6 Month Contract)

 

Southern Frontenac Community Services is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.

The Hospice Palliative Care Coordinator (HPCC) oversees a volunteer based program to deliver palliative and bereavement support throughout the communities of Frontenac County and Addington Highlands.  The Coordinator provides outreach to clients, trains and supports a team of volunteers matched to provide in-home support. The HPCC works with partner organizations to provide support programs to clients and their caregivers.

This is a 6 Month full-time contract position, 35 hours weekly until November 30, 2022. Work location may be shared between the Sydenham, ON office and a northern township location.

Primary Duties:

  • Provide recruitment, training and direction to a team of volunteers

  • Communicate with clients, provide listening support, match clients with volunteers, and make referrals to other professionals

  • Support all administrative aspects of the program, including maintenance of volunteer and client files

  • Collaborate with other organizations to establish Support Groups

  • Maintain and nurture community partnerships

Requirements:

  • A clear understanding of hospice palliative care, grief and bereavement

  • Experience managing and mentoring volunteers

  • Post-secondary education relevant to the position

  • Successful completion of a Hospice Ontario or equivalent training program

  • Comfortable with difficult conversations, including topics related to death and dying

  • Proficient with Microsoft Office programs

  • Reliable transportation and ability to travel independently throughout program catchment area

Foot Care Service Provider

(Contract - $25/hour)

 

Southern Frontenac Community Services (“SFCSC”) is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.

The Foot Care Provider is a contracted service worker engaged by SFCSC to provide basic foot care services to seniors living in South Frontenac at clinics held by SFCSC on a monthly basis.  The Foot Care Provider will provide a basic assessment, treatment and documentation with each appointment. 

This is a contract position. Work location is Sydenham, ON.

Primary Duties:

  • Attend clinics at SFCSC location(s) on the second and fourth Tuesday each month from 12:00 pm to 4:00 pm, dressed appropriately

  • Provide foot care services to clients following KFL&A Public Health and IPAC Practice Recommendations

  • Adhere and support sanitization practices between clients

  • Perform assessment and determine treatment plan for clients

  • Provide basic foot care including toenail cutting, callous reduction and application of lotion to each client

  • Document details of treatment provided, concerns and other relevant information in client file

Requirements:

  • Successful completion of a recognized Foot Care Certificate program

  • Must have own transportation

  • Successful completion of IPAC Core Competencies

  • Basic First Aid and CPR is and asset

  • Willingness to cooperate with any inspections of service

  • Willingness to attend training or meetings as requested

  • Proof of N-95 Mask Fitting

  • Proof of Full COVID-19 Vaccination

Adult Day Service Program Recreation Assistant

 (Casual $18.21/hour)

 

Southern Frontenac Community Services is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.  This is a casual position, located in Sydenham, ON.

Primary Duties:

The Adult Day Service (ADS) Program Recreation Assistant will work closely with the ADS PSW Lead and other ADS staff to plan and implement an Adult Day Service. The ADS Recreation Assistant will work with groups or provide individual support for clients with differing needs. The ADS Recreation Assistant will take the lead on recreation within the program. Physical demands of the job include assisting with transfers and client care as well as room set-up and take down.

Requirements:

  • Certificate in Therapeutic Recreation and/or Recreation & Leisure with Gerontology or equivalent. 

  • Strong Leadership skills.

  • Knowledge of the aging process as well as diseases of aging. 

  • Ability to evaluate activities and ensure that they meet both the client’s needs and interests as well as program standards of care.

Home Help Provider – Casual

Southern Frontenac Community Services (SFCSC) is a rural based organization focused on working with others in the provision of health and social services that meet the needs of our community.  This is a casual position, Monday through Friday, 8:30 a.m. – 4:30 p.m., located in Sydenham, ON

 

Duties:

  • Work closely with the Client Care Coordinator, to perform household management tasks for seniors and adults living with disabilities enabling them to remain in their homes longer.

  • Work with client or their representative to ensure understanding of the tasks they are looking to you to provide or support.  These may include (but not limited to): cleaning any room of a home, washing floors, windows, dishes, bathrooms, laundry, meal preparation, or errands (e.g. retrieving mail or grocery items). 

  • Complete requested tasks in a thorough and efficient manner, being respectful of the client and the client’s property and sensitive to their level of comfort with you in the home.

 

Qualifications & Requirements:

  • Experience providing house cleaning or household management services to others.

  • Experience interacting with seniors.

  • Two positive references.

  • Must have own transportation.

  • Commitment to comply with SFCSC confidentiality agreement.

  • Willingness to attend training or meetings as requested.
