Application Procedure - Adult Day Service
A South East Local Health Integration Network Care Coordinator (SELHIN CC) or Community Support Services Care Planner completes social, mental and functional assessments prior to determining if an individual is eligible for Southern Frontenac Community Services Corporation (SFCSC) Adult Day Program.
Having determined eligibility, the appropriate agency will make the referral and provide copies of the assessment to SFCSC.
Upon receipt of the referral and the assessments, SFCSC staff will contact the potential participant to arrange an intake visit to complete the application form and to provide information about the service.
Families may also apply directly to SFCSC who will connect them with either of the above agencies in order to complete the proper assessment paperwork.
